  • Welcome to the School District of Waukesha’s Wellness Incentive Program!  The mission of the School District’s Employee Wellness Program is to promote healthy living, decrease the risk of disease and enhance the quality of life among School District of Waukesha employees, students and their families.  By practicing healthy lifestyles, we will be role models for the student body, as well as the community at large. 

    As we mold our Wellness Program to meet the needs of our staff, we rely on aggregate data received from outside vendors.  One of the reports we receive is from Healthstat, our onsite clinic provider and vendor for the Health Risk Assessment and Biometric Screening event for our Wellness Program.  This report allows us to focus our wellness efforts on the specific health care needs of employees and their families. 

    We invite all SDW employees and their spouses who are enrolled on the District health plan to participate in the Wellness Program to further enhance our wellness efforts as a district.  This is a voluntary program, however in appreciation of your participation you will receive a lower premium share on your health and dental insurance premium costs.

    The SDW Wellness Incentive Program Requirements for the 2018 year includes 3 Steps applicable to all employees and spouses enrolled on the SDW health plan:

    1. Health Risk Assessment (HRA): The short questionnaire includes questions regarding personal history, current medications, tobacco use, vehicle safety, alcohol use and exercise.
    2. Blood draw, and height/weight measurements.  All participants will meet with a Healthstat nurse.  Biometrics will be collected at the worksite health screen and will include: height, weight, waist circumference, blood pressure, BMI (Calculated) and percent body fat.  Blood is drawn from each participant and tested for cholesterol (Total, LDL & HDL), triglycerides, and glucose. Screenings take about 10 minutes per person.
    3. Report Delivery:  All participants will have the opportunity if they wish to speak/meet with a Healthstat professional to review the results of the Health Risk Assessment and Biometrics. 

    Labwork and screening results provided from an outside provider may not be dated more than 90 days prior to the start of the 2018 onsite event to be accepted as fulfillment of the requirements.

    Notification of the Fall 2018 onsite event will be sent out in the beginning of the 2018 school year.  The requirements must be completed by both the employee and spouse(if applicable) no later than October 31, 2018 to receive/maintain the lower premium share.  Premium share changes are effective January 1, 2019.
