Newly Enrolled Members

  • You are now enrolled in your benefits with the School District of Waukesha!  If you enrolled in a health plan option between 7/1/2018 and 10/31/2018 you were automatically enrolled as a wellness participant and are enjoying a lower premium share cost.  In order to maintain the lower premium share for your coverage we invite you and your spouse if enrolled in the health plan to participate in the Wellness Program in order to maintain the lower premium cost share. This is a voluntary program and therefore participation is not required.  Please look for information this Fall further outlining the program and instructions on how to complete your screening at the upcoming onsite HRA event. If you are planning on participating, please complete the HRA and Biometric Screening by October 31, 2018.    


    Although you may not have your ID cards yet, your coverage is still effective as of your first day of work with the District.  In speaking with any providers that you may see, please let them know you will forward your insurance information when you receive it.         


    If you are newly enrolling onto the High Deductible Health Plan with the District effective 9/1/18 then you must be sure to open your HSA account at Educators Credit Union no later than AUGUST 15th in order to receive the Employer Contribution Schedule on your first payroll check in September.  Once you set up your account, Educators will automatically forward your account information to the District.  Please note that in order to avoid the $2 monthly charge on your HSA account you will need to elect electronic bank statements through Educators.

    • Download the paperwork from Educators Credit Union Partner Site
    • Return the paperwork to the Waukesha location with a copy of your picture ID
    • Or Fax to: (262)884-7235, attention: HSAs

    OR

    If you do not set up the HSA account the District cannot make any deposits on your behalf!

    Below is an outline of what to expect for your ID cards if you enrolled in any of the benefits. 

United Healthcare

  • 1.   Medical ID Cards should be arriving soon!  Your ID cards could be delayed depending on when you were hired/enrolled in your benefits.

    2.   Be sure to check the information is accurate on your ID cards

    • Check the spelling of names
    • Check all individuals are listed who should be covered on the plan
    • With the HDHP, there are NO copay amounts listed on the card

    3.   Register on MyUHC.com

Navitus Pharmacy Benefits

  • The District's Prescription Drug Coverage is with Navitus.

    1.   You will receive a separate ID Card for your prescription drug coverage. Please look for your member booklet with the ID cards attached within 7-10 business days. Your ID cards could be delayed depending on when you were hired/enrolled in your benefits.

    2.   Be sure to check the information is accurate on your ID cards

    • Check the spelling of names
    • Check all individuals are listed who should be covered on the plan

    3.   Register on www.Navitus.com

    If you need a prescription filled prior to receiving your ID card, you may contact Navitus Customer Service directly at (855) 673-6504 or the Benefits Coordinator. 

Educators Credit Union

    1. You will receive a VISA Debit Card and convenience checks for  use to pay out of pocket expenses
    2. Remember, it works like a checking account, if the money is not there it will not work

Delta Dental

    1. If you enrolled new into the Delta Dental plan, you will receive Delta Dental ID Cards
    2. These ID cards do not list covered dependents on them 
    3. To search for in-network providers visit: deltadentalwi.com

UHC Vision

  • UHC does not send out Vision ID cards. You may log on to MyUHCvision.com and print off a personalized ID card and to search for providers

Diversified Benefits Services - Flex Spending Account

    1. The new plan year begins 9/1/17 and ends 8/31/18 for all Flex accounts
    2. The Medical Flex, Limited Purpose Flex and Dependent Care are all accessible online
    3. You may register online to access your account information and file claims

Disability & Life Coverage with Lincoln Financial

    1. Long-term disability and Life coverage are provided by the District
    2. If you enrolled new onto the voluntary short-term disability coverage, there is a 12 month pre-existing clause on the coverage
