Health Insurance
SDW offers employees a choice of medical plans that provide comprehensive coverage, including prescription drug coverage. Both plans have the same coverages but have different premiums, deductibles, co-insurance and out-of-pocket costs.
High Deductible Health Plan (HDHP)
Wage Base Plan (WBP) (no spouses can be enrolled in this plan)
The medical plan provides access to physicians through the UHC Choice Plus Network. They include doctors, hospitals, and other health care providers who agree to meet strict quality standards and provide care at a discounted price. You may use a non-participating provider, however you will not receive a discounted rate for service and will pay a higher deductible, coinsurance and out of pocket. Provider listings are available at www.myuhc.com.
